Approach to Treatment: I believe in an individual and rehabilitative approach to treatment. It is important I am able to provide understanding, respect and a nurturing environment to the client. Being able to implement a treatment goal, which will increase the desired behavior, is crucial to a successful treatment plan. I plan to help teach clients communication and life skills, which will ultimately help the client, learn to make positive changes and decisions.

I have experience working in emergency medicine for seven years. I have also worked with youths and adults for over five years in community and therapeutic settings at Cascade Hall with Community Psychiatric Clinics, Community Care, Sound Mental Health as a counselor/clinician with dual diagnosed youth and adults. I currently continue my work in mental health as owner and counselor of my private firm Life Line Counselors.
I am bound by professional ethics to protect client communication as confidential in regard to their involvement in counseling services. For this reason, if you would like me to release information about the clients care to anyone; you are required to sign the “Release of Information” document that is part of the consent to treatment form. This only releases me to speak with professionals currently involved in the clients care, and excludes other specific individuals that you may want information released to. If you want information released to a schoolteacher or family member for example, you would have to fill out a separate release so that I may speak with that specific individual about the client’s care. Confidentiality has the following exceptions by law:

1) In the event of a medical emergency the emergency personnel provider may be given necessary information
2) If the event of a threat to harm to oneself or someone else, if that threat is perceived to be serious, the proper entities may be contacted. This may involve the client being threatened.
3) In the event of suspected child or elder abuse, the proper authorities may be contacted. The abuse does not have to be witnessed in order to be reported.
4) If ordered by a court of competent jurisdiction
5) If you file a complaint against Community Care or me concerning the clients care.
6) If your records are subpoenaed by an attorney in the State of Washington, they will be released unless you file for a Protective Order within 14 days of the subpoena
7) If the event of a child’s death or disability the information may be released if the client’s personal representative or the beneficiary of any insurance policy on the client’s life signs a release authorizing disclosure
8) In the event that the client reveals the contemplation or commission of a crime or harmful act the counselor may release information to the proper authorities.
9) In the case of a minor client, information indicating that the client was a victim of a crime may be released to the proper authorities
10) For the purpose of an audit by third party payers, outside funders, or state department of mental health for state licensing review.

Review of Records:
I keep a record of the health care services that I provide for 7 years at which time these records will be destroyed. You have a right to request to see and copy that record. You may also ask that I correct that record in writing, at which time I will review your request and approve or deny with explanation within 21 working days.

Notice to Clients:
As required by RCW 18.19.060, this will inform clients of Certified Advisors or Certified Counselors in the State of Washington that they may file a complaint with the Department of Health at any time they believe a Counselor has demonstrated unprofessional conduct. Counselors practicing for a fee must be registered or certified with the Department of Health and receive supervision on all caseloads’ for the protection of public health and safety. Registration of any individual with the Department does not include recognition of any practice standards, nor necessarily imply the effectiveness of any treatment. It is every client’s right to discontinue treatment at any time, with or without notice to the treatment provider.

Consumer Rights as provided by Washington State law:

1) You have the right to be treated with respect and dignity
2) You have the right to develop a plan of care and services, which meets your unique needs
3) You have the right to refuse any proposed treatment, consistent with the requirements in the Involuntary Treatment Acts, Chapters 71.05 and 71.34 RCW.
4) You have the right to receive care that does not discriminate against you, and is sensitive to your gender, race, national origin, language, age, disability, and sexual orientation.
5) You have the right to be free of sexual exploitation and harassment.
6) You have the right to review your case record.
7) You have the right to confidentiality.

9) You have the right to choose a primary care provider pursuant to WAC 275-57-110(5)
